CDC NCCCP Supplement S6 3 Environmental Approaches Educate Healthcare Providers RCCS will schedule meetings with representatives (hospital discharge planners, social workers, patient navigators) from hospital in 2 Eastern Kentucky Appalachian Area Development Districts (Big Sandy and FIVCO) to increase awareness and utilization of the Pathfinder Cancer Resource Directory for cancer patients and families. Materials will be provided to share with patients about the on-line pathfinder and contain instructions on how to access/search for services. Partnerships will be established to assess current patient navigator survivor services that are being offered in these 2 ADDs, obtain feedback on survivor needs, and identify additional survivor services that need to be added to the directory. S6 2 Environmental Approaches - Educate Public 1. KCP will expand its survivorship web page to include resources for organizations to use in educating the public about survivorship during national survivor campaigns. A resource kit will be developed that includes social media messages, promotion of national materials, and suggestions for activities to increase awareness of survivorship issues. This resource will be available in northern, central and eastern KY. RCCS will be available to assist organizations with using this resource to tailor messages/materials, plan and implement activities. 2. The RCCS in Eastern Kentucky region (Big Sandy and FIVCO Development Districts) will maintain an on-line, searchable cancer service directory (Pathfinder) that includes county information for Cancer Screening Tests/Tobacco use; Information, Referral and Navigation; Patient Assistance; Medical Care; Survivorship and Support Groups. RCCS will provide presentations at community events, meetings/activities (DCC meetings in two area development districts, Consortium meetings, etc.) promote and illustrating use of Pathfinders. New Normal Survivor Workshops KCP will conduct 2 one-day workshops for female cancer patients living in the eastern Kentucky Appalachian regions (Big Sandy and FIVCO area development districts). Community Clinical Linkages, Teach Survivors how to access and evaluate information. Self-management Skill Development - workshops will provide education, self-management skills training and group support in dealing with issues and concerns associated with making a successful transition from active patient to extended survivorship, focusing on the development of self-management skills for healthy living. As part of survivorship workshops, educational modules with demonstrations will be presented on: healthy diet, exercise, and stress reduction. RCCS's will be available to conduct group sessions to assist survivors in developing individual healthy living action plans. Environmental Approaches, Implement evidence based cancer plans that include the all stages of cancer survivorship. Workshops will provide education, self-management skills training and group support in dealing with issues and concerns associated with making a successful transition from active patient to extended survivorship, focusing on topics including: physical impact - awareness chronic disease, managing symptoms; treatment summary & survivorship care plan with templates and resources for these; psychological wellbeing for survivors; physical impact of cancer treatment, risk factors (healthy lifestyle) and need for continued screening.
