Description

The University of Kentucky Oral Health Program (KOHP) shall provide preventive, comprehensive dental services and health promotion activities at an enhanced level in order to decrease the prevalence and incidence of oral disease and conditions among the citizens of the Commonwealth. UK shall provide case management services at the UKCD North Fork Valley Community Health Center in Hazard. TOHSS Program: UKCollege of Dentistry shall provide effective case management of patients in the North Fork Valley Community Health Center in Hazard, who are participants in sealnt outreach program that needs more comprehensive restorative treatment. Expected activities include identification of patients, contact parents of children identified as needing restorative care in the sealant program, follow-up with patients if have appointment failures, reduce barriers to care.
