P 2. 'LPediatric medical services provided by Torn Young, M.D., for the Tates Creek Elementary School Center will include the following Serve as School Health Consultant for the Tates Creek Center. Provide medical supervision for the Nurse Practitioner at the Tates Creek Center. Provide pediatric outpatient primary care services 8 hours a month to include medical consultation and evaluation for children with learning disabilities and attention deficit disorders. Supervision of medical residents providing medical services. Participation at meetings of the professional multidisciplinary team, as needed for consultation on complex medical problems. cPuarrrtiiccuilpuamt.ion with health educators on the heal th education Participation in development of a comprehensive school health - H. Participation in development and implementation of programs, based on needs assessment such as asthma or obesity programs. I. Referral of students with no medical home to the Lexington- Fayette County Health Department Primary Care Center for services not available at the school clinics. J. Dental referral of students to the Health Department's dental clinic who are patients of the Health Department. K. Provides a monthly schedule of appointments with the students of the Tates Creek School Clinic to the School Clinic Coordinator /HRSA Grant Administrator. This schedule shall be sent by the first of each month. L. Will provide a monthly report with the schedule of his/her previous activities for the month. M. Collect and supply adequate data (as determined by the HRSA Grant), UDS Report statistics and other information to document services, meet report ing requirement s, and evaluate the effectiveness of the program~
