Of Pediatrics, RK J402 Lexington, KY 40536-0284 Attn: Ms. Paula Cox (hereinafter referred to as "Contractor"). NOW THEREFORE, in consideration of the mutual agreements set forth herein, the two parties hereto, intending to be legally bound hereby, agree as follows: The Health Department shall provide the administration of the Tates Creek Elementary School Center and shall be the billing management for reimbursable health services for the school-based centers. 1. 2. pediatric medical services provided by Tom Young, M.D., for the Tates Creek Elementary School Center will include the following: A. B. C. D. E. G. H. F. Pediatric outpatient primary care services, 8 hours per month. Supervision of medical residents providing medical services. Medical consultation for children with learning disabilities and attention deficit disorders. Participation at meetings of the professional multidisciplinary team, as needed for consultation on complex medical problems. Participation with health educators on the health education curriculum. Participation in development of a comprehensive school health program. Participation in development and implementation of programs, based on needs assessment such as asthrnaor obesity programs. Referral of students with no medical home to the Lexington- Fayette County Health Department Primary Care Center for services not available at the school centers. Dental referral of students to the Health Department's dental I.
