Tates Creek Elementary School Center

Pediatric medical services provided by Tom Young, M.D., for the Tates Creek Elementary School Center will include the fOllowing: A. pediatric outpatient primary care services, 8 hours per month. B. Supervision of medical residents providing medical services. C. Medical consultation for children with learning disabilities and attention deficit disorders. D. Participation at meetings of the professional multidisciplinary team, as needed for consultation on complex medical problems. E. Participation with health educators on the health education curriculum. F. participation in development of a comprehensive school health program . G. participation in development and implementation of programs, based on needs assessment such as asthma or obesity programs. H. Referral of students with no medical home to the Lexington-Fayette County Health Department Primary Care Center forservices not available at the school centers. I. Dental referral of students to the Health Department's dental clinic who are patients of the Health Department.
