Description

Background. CFP's Child and Family Services (CFS) division provides clinical case management and related services at nine CFP field offices located in five states directly to foster youth, young adults, and families impacted by the child welfare system. CFS has implemented the Child and Adolescent Needs and Strengths (CANS) and the Family Advocacy Support Tool (FAST) as part of its direct services. Purpose. Service Provider will continue to provide expertise and consultation to CFS to further staff ability to use CANS, FAST, and Transformational Collaborative Outcomes Management (TCOM), advance CFS’s Continuous Quality Improvement (CQI) framework, which includes integration of CANS/FAST/TCOM, and connect to the larger TCOM community. Outcomes. Successful completion of services will result in CFP advancing TCOM principles in practice, increased connection to CANS/FAST and TCOM collaborators, learning and presentation opportunities for CFP staff, implementation of advanced CQI processes and analysis that incorporate CANS/FAST data and increased practice skill efficacy.
