Description
This contract is established to provide for reimbursement the Contractor for community based COVID-19 testing
performed on citizens of the Commonwealth for expenses incurred during the state of emergency.
The Contractor shall:
1. Meet the following requirements:
A. Be in good standing with the Commonwealth. The Commonwealth defines good standing by having necessary and
applicable licenses and certifications for all of its facilities and employees.
B. NOT be excluded from participation in, terminated from, or under payment suspension by, any federally or state
funded health care programs, as defined in § 1128B(f) of the Social Security Act. 42 U.S.C. § 1320a–7b. This includes
but is not limited to exclusion by HHS/OIG; termination from
Medicare, Kentucky Medicaid, or any other state Medicaid program; or under payment suspension by same. No
payment shall be made under the contract with respect to any item or service furnished by such excluded, terminated,
or suspended individual or entity.
2.Provide testing to citizens of the Commonwealth with an approved Centers for Disease Control (CDC) test;
A. CDC recommends collecting and testing upper respiratory tract specimens (nasopharyngeal swab). CDC also
recommends testing lower respiratory tract specimens, if available. For patients who develop a productive cough,
sputum should be collected and tested for COVID-19.
B. Approved tests for reimbursement would follow the Center for Medicaid Services Healthcare Common Procedure
Coding System (HCPCS) or Current Procedure Terminology Code (CPT). The following codes may be billed for
reimbursement:
i. U0001 CDC supplied test
ii. U0002 Non-CDC supplied test
iii. U0003 High Output Technology Labs Only
iv. U0004 High Output Technology Labs Only
v. 87635 Non-CDC supplied test
vi. 86328 – Immunoassay for infectious agent antibody(ies), qualitative or semi quantitative, single step method
(eg, reagent strip); severe acute respiratory syndrome coronavirus 2 (SARS- CoV-2) (Coronavirus disease [COVID-19])
vii. 86769 – Antibody; severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) (Coronavirus
disease [COVID-19])
viii. G2023 – Specimen collection for severe acute respiratory syndrome coronavirus 2 (SARS- CoV-2)
(coronavirus disease [COVID-19]), any specimen source.
ix. G2024 - Specimen collection for severe acute respiratory syndrome coronavirus 2 (SARS- CoV-2) (coronavirus
disease [COVID-19]) from an individual in a SNF or by a laboratory on behalf of a HHA, any specimen source.
x. Other procedure codes approved by CMS for COVID-19 related testing.
3. Be responsible for providing the facilities, personal protective equipment and staff for the collection of the test
specimen;
4. Provide to the public the days, hours and locations for free COVID-19 testing;
5. Provide the public a way to schedule testing appointments;
6. Collect and test each specimen;
7. Notify the patient of the result;
8. Reimbursement rate of the tests are noted in Section 2.04 of this contract;
9. Track and report all costs associated with reimbursement of testing;
10. Submit with their monthly invoice a complete report of positive or negative test results; and
11. Maintain patient file as it relates to all test results, negative or positive, completed CDC and DPH required forms. DPH
reserves the right to review and audit the file as it relates to these test results and completed forms
