UK Client Satisfaction Survey

Conduct client satisfaction surveny, data analysis and report production for clients rehabilitated by the Kentucky Office for the Blind closed into successful employment and clients closed unsuccessfully for the period October 1, 2006 through September 30,2007. The survey will be designed and the data anlyzed in consultation with OFB staff and representatives of the Statewide Rehabilitation Council. Produce electronically in Microsoft Word an Executive Summary by December 17, 2008; produce data by breakdown by item for each counselor caseload by the end of the first week of January 2008 and a final formal report by January 30, 2008. These materials shall be presented to the first part at 209 Saint Clair Street, Frankfort, Kentucky or another location by agreement of the parties. Delivery of the material shall include availability to answer questions regarding the materials and/or methodology of the survey as needed by the first party. The availability includes but is not limited to presenting survey results and analysis at a designed meeting of the OFB Statewide Rehabilitation Council at a date and time to be determined in consultation between the parties.
