Re-Link Abstract
There is an estimated prevalence of 4 million people currently living with chronic Hepatitis C
Virus (HCV) infections in the United States. Of these, approximately 40% are still unaware of
their diagnosis and the majority have not received e?ective lifesaving cures despite increasing
restrictions to treatment. Within UKHC alone, from July 2018 to August 2024, there are 7,752
chronically infected HCV patients who have been diagnosed but remain untreated. As our
CURES model of care continues to grow throughout the region, which is primarily a point of care
intervention for new diagnosis, it is important that these previously diagnosed patients are
found, linked to care, and treated. This RFP will allow dedicated resources to build out an
enterprise-wide registry through a novel software called “Compass Rose” which will replace
cumbersome existing platforms currently being used to manage this patient population and
allow for more streamlined and modern approaches to be used to leverage the electronic to
improve key performance indicators along this care continuum. We believe that electronic
interventions will be key to e?ectively managing such a patient population so that we maximize
our enterprise level opportunities to engage these patients and providers caring for them.
Additionally, this information will add to a growing body of literature regarding best practices on
how to eliminate this epidemic and others.
