Description
The Cabinet for Health and Family Services, Department for Public Health (DPH), Division of Prevention
and Quality Improvement, is contracting with the University of Kentucky to provide expertise in the
facilitation and strategic planning for the development, implementation and communication of the
Coordinated Chronic Disease Prevention and Health Promotion (CCDPHP) state planning process as
required by Centers for Disease Control (CDC) Cooperative Agreement. The CDC requires that each state
develop a coordinated plan to promote and implement measures for the purpose of preventing chronic
diseases and promote better health standards and develop public health leadership skills. This contract
shall ensure that the Commonwealth of Kentucky can meet the goals and expectations of the Centers for
Disease Control Chronic Prevention and Health Promotion Plan.
