Unpuzzling Disability (UD) will create a series of three training videos on working with people who have disabilities. Trainings are to be recorded for sharing amongst Ampersand staff. Each training will be between 15-45 minutes in length and will address an issue agreed upon by Ampersand and Unpuzzling Disability by May 22, 2020. The work will be done by HDI staff and contractors who are all people with disabilities. Unpuzzling Disability will conduct an accessibility audit of the following Ampersand web and print materials to include: Brown-Stanley Safety C-SSRS-Screener Client Service Follow Up Example Volunteer Timesheet Individual Service Log Legal Advocacy Checklist Timesheet Medical Advocacy Request for Services Continuing Education Medical Advocacy Follow-up Walkthrough (video) Remote Medical Advocacy Volunteer Handbook (9 chapters) Staff Advocate Guidelines and Policy Volunteer Homework Worksheets (6 versions) Volunteer Training Presentations (13 PPT average 30 slides) Volunteer Training Videos (13) Additional Resources List Safety Plan HIPAA Client Client Orientation Email Security Form Other forms as requested by Ampersand Website (all) Measurable Goals and Objectives • Unpuzzling Disability will create 3 training videos for Ampersand staff on topics related to disability and provide captioned MP4 files for each. • UD will audit all materials submitted by Ampersand and produce a written report with suggestions for creating added accessibility. • UD will audit the Ampersand website for accessibility related to vision, hearing, intellectual disability and create a written report. • Accessible versions of forms (plain language, easy-read) will be created as appropriate. For example, client materials may need more adjustments than forms used only by staff
