School Address 8. School Principal and System Superintendent 9. Fax Number By August 1 provide the following: 1. A preliminary report that includes the items with asterisks from the evaluation sheet. Our Grants Accounting Office will not allow us to reimburse costs unless we have a preliminary report submitted on or before August I, 2004. By June 1, 2005 provide the roUowing: 1. Final Report with all data about your program including results of project and follow-up experiences with all pertinent evaluation data.
