FY27 Quality Improvement Project C6046

2.01 Deliverables UKRF shall: 1. Assist in organizing and convening community activities for the learning collaborative at least one partner and facilitate reporting quality improvement steps. 2. Assist in identifying key partners for the learning collaborative (ConnectCARE Network) community activities including those that provide social services and support. 3. Utilize practice coaches and IT specialists to provide support for participating practice and community-based teams as they develop plans to incorporate changes for improving outcomes of people with hypertension and support needs. 4. Assist partners with developing and using comprehensive customized quality measures through EHRs or HIT to identify, track and monitor referrals to social services and support needs and quality measures with the ability to disaggregate by race, ethnicity and other populations of focus. 5. Document processes, changes, and outcomes as part of the overall grant evaluation. 6. Assist with education and implementation of Plan Do Study Act (PDSA) cycles for design, development, and reporting. 7. Assist with required evaluation indicators and performance measures and collect this data from identi?ed partners.
