Regional clinical educational support service - A regional clinical educational support service for primary care physicians in the best management/prescribing practices for disease states but not limited to: Type 2 diabetes, asthma, and nonmalignant chronic pain. Patient-specific pharmacotherapy monitoring - Patient-specific pharmacotherapy monitoring for Medicaid/Passport recipients by clinical pharmacists associated with the University of Kentucky College of Pharmacy. Categories to be covered by pharmacotherapy: asthma, diabetes, and high-cost Medicaid patients.
