Money Follows the Person (MFP) - Implementation and Evaluation State

Provide state and region personnel to be based in Frankfort at the Cabinet for Health and Family Services offices. UK shall at all times assure that the professional staff positions and functions support the delivery of the MFP program, in conformance with the Kentucky MFP plan approved by the Center for Medicare and Medicaid Services (CMS). 3. Provide a staffing plan which shall consist of 3 regional teams composed of a social worker program coordinator and a nurse program coordinator to implement the MFP program at sites across the state. 4. Provide implementation of the MFP program, which shall consist of contacting and interviewing potential clients, assisting clients with an MFP application, assisting clients with home modification and household startup processes, and monitoring and evaluation once placed in a community setting. 5. Provide regional teams that shall be directed by a state management team. The state management team shall consist of a nurse program manager and a social worker program manager.
