PATient Navigator to rEduce Readmissions (PArTNER)

Dr. Williams will serve as the Project BOOST Mentor for implementing the BOOST at the University of Illinois Hospital & Health Sciences System. As BOOST Mentor, he will conduct two site visits to work with the hospital's senior leadership and the BOOST QI team and supervise at least ten approximately monthly mentor calls with the BOOST QI team. He will be actively invovled in the QI teams efforts to identify barriers to implementation and help them formulate solutions. He will also particpate in developing reports, abstracts, and peer-reviewed publications, and working with the Society for Hospital Medicine help ensure that the program will be integrated ito the SHM BOOST toolkit for dissemination
