PROJECT NARRATIVE Specific Aims This project seeks to accomplish three specific aims: 1. Implement the IHIP intervention (originally developed as an employee wellness model) in a broad sample of community primary care practices and patients. 2. Test the efficacy ofthis health behavior change program at the patient level. Test the accePtabt, adoption, and use of this program at the primary care practice and provider levels.
