Resumen

The practice of documentation provides the means by which medical speech-language pathologists inform consumers and payers about the effects of their interventions on patients' lives. Documentation practices are increasingly more controlled by health care reform practices rather than by the clinicians providing the interventions. This clinical note provides guidelines and relevant information about outcome measures and their use to assist providers in reassuming control and responsibility for their documentation.
