Resumen
Daptomycin dosing in pediatrics has not been updated since initial approval to align with contemporary adult dosing recommendations. We observed lower than anticipated exposures with median 24-hour area under the curve of 378 mg·h/L [interquartile range (IQR): 368–413] in 4 children despite use of high doses (17–20 mg/kg/d). There is a critical need to reappraise daptomycin dosing in pediatric patients for Staphylococcus aureus bloodstream infections.
