Rewriting the Note with Technology

The current landscape of physician/advanced practice provider documentation is shaped not only by the influence of 6 primary stakeholder groups but also by the unintended consequences outlined in Goodhart's Law and principles from behavioral economics and choice architecture. Together, these forces have contributed to ongoing inefficiencies and cognitive burdens in clinical documentation. The 25 x 5 Initiative, for example, is a national effort led by American Medical Informatics Association in collaboration with the AMA, launched in 2022, that aims to reduce clinical documentation burden to 25% of its current state through policy reform and HIT tool optimization.80 For meaningful reform, all stakeholders must collaboratively engage with the underlying behavioral and social science insights that drive documentation behavior. A forward-looking strategy must integrate lessons from prior systems while critically examining current practices. Only through this lens can documentation frameworks be reimagined that not only enhance clinical utility and data integrity but also promote physician well-being, patient communication, and operational efficiency.
